1. Human Umbilical Cord Mesenchymal Stem Cell Transplantation for the Treatment of Chronic Discogenic Low Back Pain

Human umbilical cord tissue-derived mesenchymal stem cells (HUC-MSCs) consists millions of stem cells and has the potential to regenerate degenerative discs. Previous animal and in vitro experiements had yielded positive results and so the researchers carried out a preliminary study with the aim of testing the safety and feasibility of the therapeutic value of HUC-MSC transplantation in patients suffering chronic discogenic low back pain. 2. Human umbilical cord-derived mesenchymal stem cells reduce monosodium iodoacetate-induced apoptosis in cartilage

The goal of the study was to investigate the therapeutic potential of mesenchymal stem cells of the human umbilical cord, and its mechanism on destruction of joint cartilage induced by monosodium iodoacetate.  Observations from the study showed that transplantation of human umbilical cord-derived mesenchymal stem cells actually proved effective in preventing MIA damage of the cartilage. 3. Umbilical cord blood stem cell treatment for a patient with psoriatic arthritis

A 56-year old was diagnosed of psoriatic arthritis based on results from clinical and laboratory tests. Standard treatments (using steroids, methotrexate, and non-steroidal anti-inflammatory drugs) did not resolve the symptoms. Some adverse effects were experienced with the addition of TNF-alpha inhibitors. Umbilical cord stem cell therapy was then put on trial. The patient was infused several times with cord blood stem cells through intra-articular and intravenous injections. Improvements were observed namely symptom remission, and normalization of serologic results for erythrocyte sedimentation rate and inflammatory markers C-reactive protein. These positive results were sustained for more than a year. The result from this study suggests that umbilical stem cells may be of great therapeutic value to patients with psoriatic arthritis, especially those who may be unable to tolerate standard treatments. 4. Short-Term Outcomes in Treatment of Knee Osteoarthritis With 4 Bone Marrow Concentrate Injections

On average, patients experienced an 84.31% decrease in resting pain, a 61.95% decrease in active pain, and a 55.68% increase in functionality score at the final follow-up. Patients also reported a mean 67% total overall improvement at study conclusion.

Introduction About the Topic

Stem cell therapy belongs to an emerging area of medicine known as Regenerative medicine. It involves the use of stem cells (which are very potent) for the repair of body tissues that have degenerated (such as the muscles, joints, ligaments, and the tendons); or rebuilding of these body parts. These stem cells may be sourced from the bone marrow or from the umbilical cord or fat tissues. All sources have rich supply of stem cells, cell signaling substances, and growth factors. The cells are drawn in a way that does not cause pain, and then injected into the affected part of the body.   PRP may be used to enhance the effects of stem cell.  Of course, ultrasound guidance is used for clear visualization. Platelet rich plasma is gotten from just a simple draw of the blood. The blood is spun in a centrifuge to separate the platelet rich component.

So what are stem cells?

The term stem cells means “undifferentiated.” What this implies is that, unlike other body cells, stem cells can grow into other kinds of cells. They have this inert ability to restore or repair the internal systems of your body. Theoretically, stem cells can help in the repair of muscles, tissues, blood, organs, and bones. It also aids growth and even replaces missing pieces 1.

Stem cells can be harvested from a number of regions including fat or adipose tissues, umbilical cord and, the bone marrow. With regards to pain management, the least invasive & convenient is that harvested from the umbilical cord. Stem cells for pain management are usually harvested from the umbilical cord 2. After collection, the stem cells are conditioned in a number of ways, like centrifuging, which potentiates the stem cells to the highest state possible. The stem cells are then injected at the affected site, with the hope that it will help the affected system or tissue to regenerate.  Fat or adipose stem cells are obtained by a mini-liposuction from the same person under local anesthesia and carefully processed.  Bone marrow stem cell can be obtained from the pelvic bone or Tibia bone under local anesthesia.

A very important concern of stem cell utilization has been the issue of safety, especially with regards to abnormal growth of cells. A lot of research has been done on this. There have been fears in some quarters that if these undifferentiated cells are injected into the body, then there may be the risk of spontaneous mutation of cells, resulting in tumor formation. While this has occurred during the utilization of embryonic stem cells, the same has not been observed with the use of stem cells from the umbilical cord, bone marrow or fat stem cells.

What roles do stem cells play in pain management?

Because they have the ability to regenerate or restore, it is logical that stem cells be applied in pain management. Stem cells have proven very effective in the treatment of disc herniation and degenerative disc disease, joint osteoarthritis & in tendon injury. Disc degeneration , joint osteoarthritis & tendon injury occurs due to injury or aging, results in the tear and weakening of the discs, joints or tendons, meaning that they cannot provide the support required by the body. The result may be herniation of the discs, joint osteoarthritis & tendon injury.

Current Treatment at Other Clinics

The current treatment methods for disc degeneration and other degenerative diseases have their drawbacks: opioids have a number of side effects, and are a bit dangerous. Surgery alters the natural structure of the body (and sometimes may be unsuccessful). Injections on its own is quite beneficial to many patients, but have little to no effect at the cellular level nor do they encourage self-repair.

Stem cells however reduce the inflammation and pain that are typical of these conditions. They also work to regenerate and repair the damaged regions 2. Current research on the effect of stem cells in pain management has shown positive results.

Stem cell treatment for elbow injury is a quick procedure. Why? Because we harvest the stem cells, and inject it into you on the same day. Complete stem cell treatment for elbow injury takes place over a course of one week but can be done in one day also. This depends on the patient’s situation. Stem cell injection for elbow injury would comprise of a pre-injection, embryological stem cell extraction, and a reinjection procedure, accompanied by post-injection of platelet rich plasma. The stem cells are injected along with their own natural growth factors. This procedure is done with advanced imaging guidance into the affected area. Imaging techniques used include musculoskeletal ultrasound and when needed real time fluoroscopy. Elbow pain can be treated with Platelet Rich Plasma (PRP) therapy. This is a non-surgical procedure, and is widely applied in the treatment of minor arthritis, muscle tear and strain, tears of the ligament and tendon, and of course, regular joint maintenance. We also apply platelet-rich therapy in the post-injection phase of stem cell treatment to boost repair of injured tissues by the stem cells.

We use an advanced form of platelet-rich therapy injection, which is less costly than the stem cell procedure, and also applied in orthopedic conditions. The platelet injection is rich in healing components that enhances the ability of the body to repair itself.

With the Piezowave 2 (which is the extracorporeal shockwave therapy that we use), sound waves of high energy are introduced into the tissue surrounding your elbow. Because scar tissue is brittle and cannot bend like healthy tissue, it breaks down with the sound waves.
