1. • Rompe, JD, et al. "Evaluation of low-energy extracorporeal shock-wave application for treatment of chronic plantar fasciitis." Jour Bone Joint Surg. 2002; 84:335-41.

From 1993 to 1995, a prospective, two-tailed, randomized, controlled, observer-blinded pilot trial was performed to assess whether three applications of 1000 impulses of low-energy shock waves (Group I) led to a superior clinical outcome when compared with three applications of ten impulses of low-energy shock waves (Group II) in patients with intractable plantar heel pain. The sample size was 112. The main outcome measure was patient satisfaction according to a four-step score (excellent, good, acceptable, and poor) at six months. Secondary out-come measures were patient satisfaction according to the four-step score at five years and the severity of pain on manual pressure, at night, and at rest as well as the ability to walk without pain at six months and five years. Three treatments with 1000 impulses of low-energy shock waves appear to be an effective therapy for plantar fasciitis and may help the patient to avoid surgery for recalcitrant heel pain. In contrast, three applications of ten impulses did not improve symptoms substantially.

Many clinical trials have evaluated the use of extracorporeal shock wave therapy for treating patients with chronic tendinosis of the supraspinatus, lateral epicondylitis, and plantar fasciitis. Although extracorporeal shock wave therapy has been reported to be effective in some trials, in others it was no more effective than placebo. The multiple variables associated with this therapy, such as the amount of energy delivered, the method of focusing the shock waves, frequency and timing of delivery, and whether or not anesthetics are used, makes comparing clinical trials difficult. Calcific tendinosis of the supraspinatus and plantar fasciitis have been successfully managed with extracorporeal shock wave therapy when nonsurgical management has failed. Results have been mixed in the management of lateral epicondylitis, however, and this therapy has not been effective in managing noncalcific tendinosis of the supraspinatus. Extracorporeal shock wave therapy has consistently been more effective with patient feedback, which enables directing the shock waves to the most painful area (clinical focusing), rather than with anatomic or image-guided focusing, which are used to direct the shock wave to an anatomic landmark or structure.

Introduction About the Topic

Shockwave therapy is widely applied in urology, sports medicine, and veterinary medicine. It restores mobility and relieves the individual of pain. As a non-surgical therapy, shockwave is non-invasive and does not require painkillers, thus making it an ideal therapy to facilitate recovery from various conditions that cause chronic or acute pain.

How it Works

There is a set standard for applying shockwave therapy. The individual’s medical history will be examined by the podiatrist. Doing this will enable him or her to isolate the affected area. It is important to know the condition to be treated, and whether or not it can be treated with shockwave therapy.

While examining the patient, the specialist will be able to locate the area of intense pain. A water-based medium is applied to this spot. This enhances impulse transmission to the affected areas. A probe is then placed on top of the affected area, and this marks the beginning of the treatment for joint pain (or any other condition).

How long does it take for the shockwave therapy to work?

Shockwave therapy can be concluded in 3-4 sessions, with each session taking roughly 15-30 minutes. This also depends on the severity of the condition. We encourage you to visit our facility so as to get the correct diagnosis and of course, the right plan of treatment.

If you have a condition such as patellar tendonitis, then it is important that you co-operate with the podiatrist that we will assign to you so that you can maintain the required exercises for the condition – such as strength exercises, and balancing exercises.

What is the duration of treatment?

Shockwave therapy causes a drastic improvement in the symptom by reducing pain while enhancing the function of your patellar tendon. The shockwave therapy usually gets rid of the tendonitis.
While surgery guarantees a 75% success rate in a space of 18 months, with shockwave therapy, the same condition can improve by 80% within the same time range. Obviously, shockwave therapy is better than surgery with regards to treatment of tendinopathy.

Shockwave therapy is very effective and safe and yields very positive results in the treatment of patellar tendinopathy.

Cons/side effects

Shockwave therapy can be expensive but we offer a fair price at Los Angeles County. What is involved in the procedure?

ESWT is beneficial to patients because they receive extracorporeal treatment for their condition, meaning outside the body. Because ESWT is non-invasive, the recovery period is not lengthy, meaning no risk of causing further damage. We cannot say the same with surgery.

During the ESWT treatment, you will lie on your back while the area to be treated will rest on the machine, on a soft membrane filled with water. An ultrasound will be used to scan and view the target location. You will hear some clicking sounds during the treatment. Our physicians will ensure that you are comfortable during treatment.

What are the conditions that can be treated by this therapy?

ESWT can effectively treat a range of musculoskeletal conditions. Complaints involving attachment points for tendons and ligaments in major joints such as the plantar fascia, Achilles tendon, shoulder, elbow and knee are common areas for ESWT treatment.

What is the recovery rate?
