Biologics may help to optimize the healing environment after rotator cuff repair. Mesenchymal stem cells (MSCs) may have the potential to regenerate a physiological enthesis, thereby improving healing at the repair site after rotator cuff repair. The current literature regarding therapeutic use of MSCs in shoulder surgery is limited. Although in vivo animal studies have shown some promising approaches to enhance tendon-to-bone healing, the use of MSCs for shoulder surgery should still be regarded as an experimental technique. Further basic and clinical research is needed until a procedure can be defined for the routine use of these cells in shoulder surgery.

Tendon stem/progenitor cells (TSPCs) were recently identified within tendon tissues. The aim of this study was to investigate TSPC-seeded knitted silk-collagen sponge scaffold for functional shoulder repair. The multidifferentiation potential, proliferation, and immune properties of TSPCs were investigated in vitro, while the efficacy of TSPC-seeded knitted silk-collagen sponge scaffolds in promoting rotator cuff regeneration was evaluated in vivo within a rabbit model. TSPCs, which exhibited universal stem cell characteristics (i.e., clonogenicity, high proliferative capacity, and multidifferentiation potential), nonimmunogenicity, and immunosuppression, proliferated well on our scaffold in vitro. Implantation of allogenous TSPC-seeded scaffolds within a rabbit rotator cuff injury model did not elicit an immunological reaction, but instead increased fibroblastic cell ingrowth and reduced infiltration of lymphocytes within the implantation sites at 4 and 8 weeks postsurgery. After 12 weeks, the allogenous TSPC-treated group exhibited increased collagen deposition and had better structural and biomechanical properties compared to the control group. This study thus demonstrated that the allogenous TSPC-seeded knitted silk-collagen sponge scaffold enhanced the efficacy of rotator cuff tendon regeneration by differentiating into tenocytes, and by secreting anti-inflammatory cytokines that prevent immunological rejection. Hence, allogenous TSPC-seeded knitted silk-collagen sponge scaffolds can be a clinically useful application for tendon tissue engineering.

Recent studies report a relatively high failure rate for tendon-bone healing after rotator cuff repair. Several studies have investigated biologically augmented rotator cuff repair; however, none has shown the application of synovial mesenchymal stem cells for such repair. Forty-two patients with a diagnosed rotator cuff tear preoperatively were enrolled in this study. Human mesenchymal tissues were obtained during arthroscopic surgery for rotator cuff tears from 19 donors who met the inclusion criteria and had investigable amounts of tissue. Colony-forming units, yield obtained, expandability, differentiation potential, epitope profile, and gene expression were compared among the cells from 4 shoulder tissues: synovium of the glenohumeral joint, subacromial bursa, margin of the ruptured supraspinatus tendon, and residual tendon stump on the greater tuberosity (enthesis). The findings indicate that the subacromial bursa is a good candidate for the source of mesenchymal stem cells in rotator cuff tears.

Above are AB’s pre and post MRI’s.  On the left the rotator cuff tendon (red arrows) are bright in color and mottled in appearance. This means that it’s a full thickness tear with severe degeneration.  On the right is AB’s MRI 2 year post stem cell injection.  The rotator cuff tendon identified by the yellow arrows is better organized and darker in color consistent with significant healing.    This is consistent with her clinical improvement.

Introduction About the Topic

Research has shown that most rotator cuff surgeries do not improve the patient’s condition. It is believed that some patients are better off with no surgery at all. Rotator cuff tear

The rotator cuff refers to a group of muscles whose role it is to stabilize the shoulder. They enhance mobility. Whenever you move your shoulder, or attempt to move it, you are putting your rotator cuff to use. This helps to stabilize and move your joint.

The rotator cuff is very prone to injuries. Injuries that are very common to the rotator cuff include bursitis, tendinitis, and strains.

Injuries to the rotator cuff may be mild or severe and are placed into any of three categories.

Tendinitis occur when the rotator cuff is overused. This results in inflammation. It is very common in tennis players and painter.

Bursitis is also another common injury to the rotator cuff. Bursitis basically refers to the inflammation of the bursa. The bursa are sacks filled with fluid. These sacs lie between the tendon of the rotator cuff and the underlying bone.

Strains or tears to the rotator cuff occur in the event of an acute injury or during overuse. The tendon may undergo a partial or complete tear. Other causes of a rotator cuff tear include automobile accident, and a fall or an acute injury.

Shoulder Labrum Tear

The labrum is a cartilage located in the shoulder’s socket joint. It cups the joint at the upper arm bone, thus linking the two joints.

The labrum positions the ball in the socket with the help of the rotator cuff. This allows the rotation of the upper arm. Consistent injuries or repetitive motions can cause a tear in the labrum, resulting in pain.

There is also a labrum at the point where the leg meets with the hip. However, a tear in this region is rare because of its thickness.

The shoulder also has a shallow opening known as the glenoid. This opening has a socket-like appearance. Tears may happen anywhere around here.

Labrum tears are of three types:

  • Bankart tear
  • SLAP tear or lesion
  • Posterior labrum tear

Labrum tears may also result from traumatic injury, and tear due to repetitive motion.

Shoulder osteoarthritis

There are two important joints in the shoulder – the glenohumeral joint, and, the acromioclavicular joint. At the acromioclavicular joint, there is the meeting of the clavicle (or collarbone) with the acromion. At the glenohumeral joint, the humerus meets with the scapula. Osteoarthritis is prominent in the AC joint.

Osteoarthritis occurs in persons above the age of 50. It may occur in young people, but due to external factors such as dislocation, fracture (of the shoulder).  This is referred to as posttraumatic arthritis.

The primary symptom is pain. Pain occurs while moving the shoulder or after moving the shoulder. The pain may even occur during sleep.

Another symptom is a reduced range of motion. This is evident when the patient attempts to move his or her arm. An attempt to move the shoulder may result in a creaking or clicking noise.

Current Treatment at Other Clinics

Current treatment of shoulder involves using medications for pain, steroid injection, shoulder surgery and physical therapy which can temporary alleviate the pain.   These treatments are considered a patch up of the problem without treating the root cause of the problem.  Medications & steroids have many side effects including weight gain & increased blood sugar from steroids, damage to kidneys, heart, stroke & gastric ulcer from NSAIDs.  Surgery can be very debilitating with prolong downtime during recovery requiring at times prolonged bed rest & even residing in nursing home for recovery.  It also entails a lot of risk including risk of infection, bleeding, poor surgical outcome to name a few.  At many times surgery fails to alleviate the pain.  Stem cells however, reduce the inflammation and pain that are typical of these conditions. Does stem cell therapy have any side effects?
