Autologous plateletrich plasma (PRP) therapies have seen a dramatic increase in breadth and frequency of use for orthopaedic conditions in the past 5 years. Rich in many growth factors that have important implications in healing, PRP can potentially regenerate tissue via multiple mechanisms. Proposed clinical and surgical applications include spinal fusion, chondropathy, knee osteoarthritis, tendinopathy, acute and chronic soft-tissue injuries, enhancement of healing after ligament reconstruction, and muscle strains. However, for many conditions, there is limited reliable clinical evidence to guide the use of PRP. Furthermore, classification systems and identification of differences among products are needed to understand the implications of variability.

Plateletrich plasma is defined as autologous blood with a concentration of platelets above baseline values. Plateletrich plasma has been used in maxillofacial and plastic surgery since the 1990s; its use in sports medicine is growing given its potential to enhance muscle and tendon healing. In vitro studies suggest that growth factors released by platelets recruit reparative cells and may augment soft-tissue repair. Although minimal clinical evidence is currently available, the use of plateletrich plasma has increased, given its safety as well as the availability of new devices for outpatient preparation and delivery. Its use in surgery to augment rotator cuff and Achilles tendon repair has also been reported. As the marketing of plateletrich plasma increases, orthopaedic surgeons must be informed regarding the available preparation devices and their differences. Many controlled clinical trials are under way, but clinical use should be approached cautiously until high-level clinical evidence supporting plateletrich plasma efficacy is available.

3. Kajikawa Y, Morihara T, Sakamoto H, Matsuda K, Oshima Y, Yoshida A, Nagae M, Arai Y, Kawata M, Kubo T. Platelet-rich plasma enhances the initial mobilization of circulation-derived cells for tendon healing.

Circulation-derived cells play a crucial role in the healing processes of tissue. In early phases of tendon healing processes, circulation-derived cells temporarily exist in the wounded area to initiate the healing process and decrease in number with time. We assumed that a delay of time-dependent decrease in circulation-derived cells could improve the healing of tendons. In this study, we injected platelet-rich plasma (PRP) containing various kinds of growth factors into the wounded area of the patellar tendon, and compared the effects on activation of circulation-derived cells and enhancement of tendon healing with a control group (no PRP injection). To follow the circulation-derived cells, we used a green fluorescent protein (GFP) chimeric rat expressing GFP in the circulating cells and bone marrow cells. In the PRP group, the numbers of GFP-positive cells and heat-shock protein (HSP47; collagen-specific molecular chaperone)-positive cells were significantly higher than in the control group at 3 and 7 days after injury. At the same time, the immunoreactivity for types I and III collagen was higher in the PRP group than in the control group at early phase of tendon healing. These findings suggest that locally injected PRP is useful as an activator of circulation-derived cells for enhancement of the initial tendon healing process.

Introduction About the Topic

Platelet rich plasma is a well-known form of treatment. It has found wide application in a variety of orthopedic conditions. Platelet rich plasma refers to platelets concentrated from your blood. These platelets are laced with growth factors that facilitate the healing of injuries.  Growth factors trigger the healing response in the human body. When platelet rich plasma is injected into the affected area (inured areas), it stimulates the ability of your body to heal and repair tissues.

Platelet rich plasma has been employed in the healing of wounds for many decades. It also helps in stimulation of bone formation during spinal fusion surgery. Of recent, platelet rich plasma has been employed in outpatient settings for the treatment of such conditions as:

  • Patellar tendonitis
  • Achilles tendonitis
  • Rotator cuff tendonitis, and
  • Tennis elbow
  • Joint Osteoarthritis including knee, hip and shoulder Osteoarthritis
  • Meniscus Injury or Tear

While some physicians treat almost any condition with PRP, others are more selective. At our facility, we examine your medical history, and your symptoms to be sure whether or not to use PRP therapy.

The Procedure

Platelet rich plasma injections are very easy and can be done at our facility. Basically, the blood is drawn, and then processed in a centrifuge. After spinning in the centrifuge, it is injected into the affected area.

Obtaining the PRP

Platelet rich plasma is derived from your own blood. The blood is drawn from your vein and then placed into a centrifuge. The centrifuge will be spun at high speed to separate the blood into different fractions. Our specialists will then extract the platelet rich fraction and inject it into the affected area.

Injecting the PRP

Platelet rich plasma injections are administered immediately after the platelets are separated from the spun blood sample. In some cases, calcium chloride or thrombin, both activating factors, may be added to the platelet concentrate.

Depending on your condition and the specialist decision, injection may be done just once, or in a series, spanning several weeks.

Side effects

Side effects of platelet rich plasma therapy are not common. However, the do occur in some cases. There is a risk of infection whenever a needle passes through the skin. However, we ensure that our needles are new and well-sterilized. Another common side effect is slight pain and inflammation after administering the PRP.

Platelet rich plasma injections are contraindicated in people who bleed, or are taking blood thinning medications (such as Coumadin). It is also contraindicated in pregnant women, cancer patients, and patients with an infection.

Current Treatment at Other Clinics

Current treatment of knee involves using medications for pain, steroid injection, knee surgery and physical therapy which can temporary alleviate the pain.   These treatments are considered a patch up of the problem without treating the root cause of the problem.  Medications & steroids have many side effects including weight gain & increased blood sugar from steroids, damage to kidneys, heart, stroke & gastric ulcer from NSAIDs.  Surgery can be very debilitating with prolong downtime during recovery requiring at times prolonged bed rest & even residing in nursing home for recovery.  It also entails a lot of risk including risk of infection, bleeding, poor surgical outcome to name a few.  At many times surgery fails to alleviate the pain.  Stem cells however, reduce the inflammation and pain that are typical of these conditions. They also work to regenerate and repair the damaged regions 2. Current research on the effect of stem cells in pain management has shown positive results.

How is platelet rich plasma therapy performed?

Blood will be drawn from a vein in your arm, and then placed in a special centrifuge. The centrifuge then spins the blood, separating the platelets from the other blood components. The effects of the platelets are enhanced by mixing it with a special substrate. The entire mix is then injected into the affected area. After healing, the pain lessens.

Is PRP painful?

How many injections will I need?

There is no specific answer to this. It will determined by your physician and the severity of your condition. Most people benefit from 1-3 treatments.

How long does a PRP session last?

Typically, it takes between 30-45 minutes to complete a PRP session.

How do I Know if PRP is effective?

Unlike other form of treatments, PRP will not mask the pain or symptoms. Rather, you will feel a great difference in your overall function and pain level. Most patients may experience an increase in pain level at the first instance, but as the healing occurs, the symptoms will resolve on their own, usually within 4-6 weeks.

What is the success rate for PRP?

This varies considerably. The success rate depends on many factors namely, the condition that is being treated, the patient’s health status, treatment protocol, and age of the patient. Success also depends on the patient’s perception. Success rates reported in the literature have been largely anecdotal. PRP success in the treatment of tennis elbow (lateral epicondylitis) has been reported in up to 80-90% in some studies, Achilles tendonitis 70-80% and golfer’s elbow (medial epicondylitis) 60-70%.
