Clinical Trials of Adult Stem Cell Therapy for Peripheral Artery Disease

Peripheral artery disease (PAD) refers to noncoronary vascular disease affecting the peripheral arteries. Most commonly the term is applied to occlusive arterial disease affecting the limb arteries, typically due to atherosclerosis. Preclinical studies indicate that a variety of stem cell therapies provide growth factors and cytokines for therapeutic angiogenesis. Small clinical trials with bone marrow mononuclear cells, as well as other cell types, have shown promise. However, mechanisms of therapeutic effect, if any, are not understood. Cell Therapy of Peripheral Arterial Disease: From Experimental Findings to Clinical Trials

The age-adjusted prevalence of peripheral arterial disease in the US population was estimated to approach 12% in 1985, and as the population ages, the overall population having peripheral arterial disease is predicted to rise. The clinical consequences of occlusive peripheral arterial disease include intermittent claudication, that is, pain with walking, and critical limb ischemia (CLI), which includes pain at rest and loss of tissue integrity in the distal limbs, which may ultimately lead to amputation of a portion of the lower extremity. The risk factors for CLI are similar to those linked to coronary artery disease and include advanced age, smoking, diabetes mellitus, hyperlipidemia, and hypertension. The worldwide incidence of CLI was estimated to be 500 to 1000 cases per million people per year in 1991. The prognosis is poor for CLI subjects with advanced limb disease. One study of >400 such subjects in the United Kingdom found that 25% required amputation and 20% (including some subjects who had required amputation) died within 1 year. In the United States, ≈280 lower-limb amputations for ischemic disease are performed per million people each year. The first objective in treating CLI is to increase blood circulation to the affected limb. Theoretically, increased blood flow could be achieved by increasing the number of vessels that supply the ischemic tissue with blood. The use of pharmacological agents to induce new blood vessel growth for the treatment or prevention of pathological clinical conditions has been called therapeutic angiogenesis. Since the identification of the endothelial progenitor cell in 1997 by Asahara and Isner, the field of cell-based therapies for peripheral arterial disease has been in a state of continuous evolution. What is peripheral artery disease?

Peripheral Artery Disease, abbreviated PAD is a disorder of the blood vessels that lie outside the brain and the heart. Peripheral artery disease is caused by the accumulation of fat in the arteries. Peripheral artery disease is also referred to as peripheral vascular disease or peripheral arterial disease.

Blood vessels that are affected by peripheral artery disease are often constricted (that is, they get narrow). This reduces the flow of blood to the stomach, kidneys, the arms, and of course, the legs.

A survey has shown that about 8.5 million American citizens are affected by peripheral artery disease. Also, about 10-20% of US citizens above the age of 60 are affected by the disease.

Peripheral artery disease contributes to stroke and heart attack. The prevalence of the disease is higher in African-Americans compared to other races. It is also more prevalent in men than in women. Smokers are also at high risk of the disease.

The symptoms of peripheral artery disease can be improved upon by physical activity.

How do i know that i have peripheral artery disease?

Research has shown that up to 50% of those affected by the disease do not even know that they have the condition. This is because most patients do not have any symptoms. The symptoms of this disorder include:

  • Loos of hair on the forelimbs and the hind limbs
  • Weakness of the legs
  • Pains in the calf and thigh muscles when climbing the stairs or when walking. This is known in clinical terms as intermittent claudication.
  • A cold feeling in the lower leg or in the foot
  • The legs feel numb
  • Slow growth of the toenails
  • Brittle toenails
  • Ulceration of the feet and the legs. This usually takes some time to heal.
  • A bluish tint or paleness of the skin of the legs.
  • Some men may experience an ED (impotence)

What are the causes of peripheral artery disease?

Peripheral artery disease is mostly caused by atherosclerosis. Atherosclerosis is not an acute condition. It occurs over time, and involves the accumulation of fatty substances inside the arteries.

Some minor risk factors for peripheral artery disease include limb injury, blood clots in the arteries, and a distorted anatomy of the ligaments and the limbs.

Other risk factors include smoking, diabetes, high blood pressure, obesity, and a high level of homocysteine or C-reactive protein.

Peripheral artery disease can be diagnosed through following tests:

  • Computed tomographic angiography
  • Ankle-brachial index
  • Doppler and ultrasound imaging
  • Magnetic resonance angiography

If this condition is not diagnosed or treated on time, it may result in very risky complications such as the loss of a leg, carotid atherosclerosis, or a high risk of coronary artery disease.

Current Treatment at Other Clinics

Recommended guidelines by the American College of Cardiology Foundation and the American Heart Foundation for the management of peripheral artery disease include:

  • Changes and adjustment in diet
  • Regular physical activity
  • Medications such as Cilostazol and pentoxifylline for those with intermittent claudication, and Ramipril, and ACE inhibitor that improves walking.

Current treatment involves using medications and at times injection.  These treatments are considered a patch up of the problem without treating the root cause of the problem.  Medications & steroids have many side effects including weight gain & increased blood sugar from steroids, damage to kidneys, heart, stroke & gastric ulcer.  Surgery can be very debilitating with prolong downtime during recovery requiring at times prolonged bed rest & even residing in nursing home for recovery.  It also entails a lot of risk including risk of infection, bleeding, poor surgical outcome to name a few.  Stem cells however, reduce the inflammation and pain that are typical of these conditions. Current ongoing research on the effect of stem cells has shown positive results.

We are different from many other clinics who offer stem cell or PRP.   Other may also not offer option of bone marrow derived stem cell, Adipose derived stem cell & umbilical cord stem cell.  They in effect expose patients to more invasive treatments like drilling bones without offering other easier options.  They also may use xray with unnecessary radiation for injections that can be easily avoided in most cases by using ultrasound.  Furthermore, they may not have proper knowledge of how other modalities can enhance the effects of Stem Cells.  These other modalities are used by us to focus on the whole body and treating different parts of it instead of just treating one out of several diseased structures in the body.

Can peripheral artery disease be treated with stem cells?

What is peripheral artery disease?

Peripheral artery disease is defined by the American Heart Association as the constriction of arteries that leads from the heart to the arms, stomach, arms, and legs. The legs are mostly affected by this disease. The major cause of PAD is accumulation of plaque in the arterial walls.

Is peripheral artery disease a long-lasting condition?

Yes! Peripheral artery disease is a chronic condition, because it does not present immediately and also persists after treatment.

What causes the pain during walking?

For your muscles to function efficiently, it need blood. If the flow of blood is restricted (as it occurs during PAD), then your muscles will be deprived of oxygen. This will cause the pain. The pain reduces when you stop using the muscles because the demand for blood has reduced.

Is it possible for people to have this disease and not know about it?

A study by the National Center for Biotechnology Information has shown that patients with PAD do not have symptoms in most cases. As such, they are not aware that they have the condition.
