Emerging Treatment Approaches for Parkinson’s disease

Parkinson’s disease (PD) is the second most common neurodegenerative disease, manifesting as a characteristic movement disorder with a number of additional non-motor features. The pathological hallmark of PD is the presence of intra-neuronal aggregates of α-synuclein (Lewy bodies). The movement disorder of PD occurs largely due to loss of dopaminergic neurons of the substantia nigra, resulting in striatal dopamine depletion. There are currently no proven disease modifying treatments for PD, with management options consisting mainly of dopaminergic drugs, and in a limited number of patients, deep brain stimulation. Long-term use of established dopaminergic therapies for PD results in significant adverse effects, and there is therefore a requirement to develop better means of restoring striatal dopamine, as well as treatments that are able to slow progression of the disease. A number of exciting treatments have yielded promising results in pre-clinical and early clinical trials, and it now seems likely that the landscape for the management of PD will change dramatically in the short to medium term future. Here, we discuss the promising regenerative cell-based and gene therapies, designed to treat the dopaminergic aspects of PD whilst limiting adverse effects, as well as novel approaches to reducing α-synuclein pathology. Treatment of Parkinson’s disease through Personalized Medicine and Induced Pluripotent Stem Cells

Parkinson’s disease (PD) is an intractable disease resulting in localized neurodegeneration of dopaminergic neurons of the substantia nigra pars compacta. Many current therapies of PD can only address the symptoms and not the underlying neurodegeneration of PD. To better understand the pathophysiological condition, researchers continue to seek models that mirror PD’s phenotypic manifestations as closely as possible. Recent advances in the field of cellular reprogramming and personalized medicine now allow for previously unattainable cell therapies and patient-specific modeling of PD using induced pluripotent stem cells (iPSCs). iPSCs can be selectively differentiated into a dopaminergic neuron fate naturally susceptible to neurodegeneration. In iPSC models, unlike other artificially-induced models, endogenous cellular machinery and transcriptional feedback are preserved, a fundamental step in accurately modeling this genetically complex disease. In addition to accurately modeling PD, iPSC lines can also be established with specific genetic risk factors to assess genetic sub-populations’ differing response to treatment. iPS cell lines can then be genetically corrected and subsequently transplanted back into the patient in hopes of re-establishing function. Current techniques focus on iPSCs because they are patient-specific, thereby reducing the risk of immune rejection. The year 2018 marked history as the year that the first human trial for PD iPSC transplantation began in Japan. This form of cell therapy has shown promising results in other model organisms and is currently one of our best options in slowing or even halting the progression of PD. Here, we examine the genetic contributions that have reshaped our understanding of PD, as well as the advantages and applications of iPSCs for modeling disease and personalized therapies. Introduction About the Topic

Parkinson’s disease is a disorder of movement. The main system that is affected by this disorder is the nervous system. Parkinson’s disease is a chronic disease because the symptoms gets worse over time.

 

There are many other disorders that affects one’s movement. These include ataxia, cerebral palsy, and of course, Tourette syndrome. These disorders occur when an individual’s nervous system undergoes a change. By this, the person’s ability to stay still or to move becomes affected.

A study by the National Institutes of Health (NIH) has shown that at least 50,000 persons are diagnosed of Parkinson’s disease in the United States every year. What’s more? At least 500,000 persons are already living with the condition.

It is important to note that the symptoms of Parkinson’s disease do not occur all at once. As a matter of fact, they have a gradual development. The first symptom is often a slight tremor and a stiff feeling in the body.

With time, other symptoms show up. Some patients do have dementia.

Majority of the symptoms of Parkinson’s disease are caused by a reduction in the levels of dopamine. Dopamine is produced in the brain.

A 2015 study conducted in France has shown that men are kore at risk of developing Parkinson’s disease compared to the female gender. However, women have a higher risk as they get older.

In many patients. The first symptoms show up from 60 years of age or above. Studies have shown that in 5-10% of cases, the symptoms came up much earlier than usual. This is referred to as early-onset PD.

How do i know that i have parkinson’s disease? What are the symptoms?

The early symptoms of Parkinson’s disease include:

  • Tremor in the hands
  • An inability to coordinate or balance oneself. The patient cannot hold an object steadily. More often than not, it falls out of the patient’s hands.
  • A change in posture or gait. The affected patient tends to lean forward slightly, as if they are in a hurry.
  • The facial expression changes and assumes a kind of fixed state. This is attributed to the changes that control the muscles of the face.
  • The voice also undergoes a change. You will notice a tremor in the voice, or it may become softer than usual.
  • A small or cramped handwriting
  • A distorted sense of smell and issues with sleep.

Other symptoms that are typical of Parkinson’s disease include:

  • skin problems
  • constipation
  • sleep problems
  • difficulty in urinating
  • Problems with chewing or swallowing.

What actually causes parkinson’s disease?

The actual cause of Parkinson’s disease is not fully understood. However, the disease comes up when the brain cells die off.

Pathological conditions that may contribute to the onset of Parkinson’s disease include:

  • Reduced dopamine levels
  • Low levels of norepinephrine
  • The presence of Lewy bodies
  • Genetic factors and autoimmune factors

WHAT ARE THE CURRENT TREATMENTS FOR PARKINSON’S DISEASE?

We understand that most features of Parkinson’s disease are caused by death of dopamine-secreting neurons. Thus, current treatment is channeled at replacing dopamine or administering drugs that mimic its functions. Such drugs include dopamine agonists or L-dopa. The bad news is that these drugs do have their own side effects which include dyskinesias (involuntary movement). It also causes neuropsychiatric problems. Thus, the drugs may ultimately become a problem to the individual.

As the disease gets more complicated, surgical stimulation of the brain tissue may be used. However, this only offers temporary relief.
