1. Davatchi F, et al. (2016). Mesenchymal stem cell therapy for knee osteoarthritis: 5 years follow-up of three patients.

Osteoarthritis is a degenerative joint disease characterized by the destruction of joint cartilage. Mesenchymal stem cells (MSCs) are found in low numbers in normal cartilage, mainly in the superficial layer, acting as repairing agents. In OA, MSCs are seen in larger numbers, but act chaotic and are unable to repair the cartilage. The synovial membrane becomes inflamed and interacts with the cartilage. Transplanted MSC have the ability to normalize them, redirecting them to their normal function. In a preliminary study, we showed that MSC could improve knee OA in four patients at 6 months. 2. Mesenchymal stem cell injections improve symptoms of knee osteoarthritis.

The purpose of this study was to evaluate the clinical and imaging results of patients who received intra-articular injections of autologous mesenchymal stem cells for the treatment of knee osteoarthritis. The study group comprised 18 patients (6 men and 12 women), among whom the mean age was 54.6 years (range, 41 to 69 years). In each patient the adipose synovium was harvested from the inner side of the infrapatellar fat pad by skin incision extension at the arthroscopic lateral portal site after the patient underwent arthroscopic debridement. After stem cells were isolated, a mean of 1.18 × 10(6) stem cells (range, 0.3 × 10(6) to 2.7 × 10(6) stem cells) were prepared with approximately 3.0 mL of platelet-rich plasma (with a mean of 1.28 × 10(6) platelets per microliter) and injected into the selected knees of patients. Clinical outcome was evaluated with the Western Ontario and McMaster Universities Osteoarthritis Index, the Lysholm score, and the visual analog scale (VAS) for grading knee pain. We also compared magnetic resonance imaging (MRI) data collected both preoperatively and at the final follow-up. The results of the study was encouraging and show that intra-articular injection of infrapatellar fat pad-derived mesenchymal stem cells is effective for reducing pain and improving knee function in patients being treated for knee osteoarthritis. 3. Platelet-rich plasma: Intra-articular knee injections produced favorable results on degenerative cartilage lesions

Platelet-rich plasma (PRP) is a natural concentrate of autologous blood growth factors experimented in different fields of medicine in order to test its potential to enhance tissue regeneration. The aim of our study is to explore this novel approach to treat degenerative lesions of articular cartilage of the knee. One hundred consecutive patients, affected by chronic degenerative condition of the knee, were treated with PRP intra-articular injections (115 knees treated). The procedure consisted of 150-ml of venous blood collected and twice centrifugated: 3 PRP units of 5 ml each were used for the injections. Patients were clinically prospectively evaluated before and at the end of the treatment, and at 6 and 12 months follow-up. IKDC, objective and subjective, and EQ VAS were used for clinical evaluation. Statistical analysis was performed to evaluate the significance of sex, age, grade of OA and BMI. A statistically significant improvement of all clinical scores was obtained from the basal evaluation to the end of the therapy and at 6-12 months follow-up (P < 0.0005). The results remained stable from the end of the therapy to 6 months follow up, whereas they became significantly worse at 12 months follow up (P = 0.02), even if still significantly higher respect to the basal level (P < 0.0005). The preliminary results indicate that the treatment with PRP injections is safe and has the potential to reduce pain and improve knee function and quality of live in younger patients with low degree of articular degeneration. 4. Extracorporeal shockwave therapy for the treatment of knee osteoarthritis: A retrospective study

This retrospective study investigated the effect and safety of extracorporeal shockwave therapy (ESWT) for treatment of knee osteoarthritis (KOA).In this retrospective study, 105 patients with KOA were included. Of those, 60 patients underwent ESWT, whereas 45 patients received laser therapy. Effect was measured by the Numeric Rating Scale (NRS) and Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC). In addition, adverse events (AEs) were also recorded in this study. All outcome measurements were evaluated at the end of weeks 1, 6, and 12.Compared with the laser therapy, ESWT showed greater effect in KOA symptoms relief with regard to NRS, WOMAC total, and its subscores at week 6 (P < .05) and week 12 (P < .01) after treatment. No AE, however, occurred in both groups.The results of this retrospective study found that ESWT may be efficacious and safe for the treatment of patients with KOA. It, however, had an intrinsic limitation as a retrospective study. Prospective study with larger sample size is still needed to warrant the result of this study in the future. Introduction About the Topic

The joint cartilage allows the joint surface to move slowly, the bones are also protected from friction. This allows absorption of shock up to 20 times the body’s weight. It enhances physical movement, and this is of great importance in athletes.

A major degenerative disorder of knee joint is osteoarthritis. It causes the joint cartilage to deteriorate overtime.

Osteoarthritis may also be caused by knee injury, such as a fracture, damage to the tendon, or tear of the ligament.

When a joint is damaged, it becomes unstable, and this results in a wearing away of the articular cartilage. The bone is then subjected to further damage, in addition to the tendon, ligaments, muscles, and lining of the synovial joint.

Knee-based stem cell therapy is slightly invasive. It is a very effective procedure that can reduce inflammation, and repair all damage that may be caused by arthritis, and equally prevent or delay knee replacement surgery.

ACL Tears

ACL is an acronym for anterior cruciate ligament. The ACL is one of the knees main ligament. It prevents the knee joint from undergoing excessive movements. The anterior cruciate ligament is located beneath the knee cap.

AN ACL tear is in most causes caused by a sport-related injury. It also happens during automobile accidents, injuries sustained while working, during rough play or falls.

ACL tears also happen when landing or pivoting from a jump. The knees give way from under the athlete when the anterior cruciate ligament gets torn.

Knee osteoarthritis

Knee osteoarthritis is a condition that affects the cartilage, the synovium of the knee joint, and the bones.

The cartilage allows easy movement for the joint surface and also cushions the bones.

The synovium acts a lining for the joint. It is a soft tissue and produces what is known as the synovial fluid. The synovial fluid is responsible for the lubrication of the knee joint. It also supplies the cartilage with oxygen and nutrients.

A breakdown of these functions causes loss of protection of the knee joints, resulting in bone damage.

Knee osteoarthritis causes stiffness and pain. Symptoms gets worse with time.

Meniscus injuries

The meniscus is a cartilage that cushions the tibia and the femur. Each knee joint has two menisci.

Damage to the menisci occur during intense physical activities that pressurizes the knee joint, or causes it to rotate. A meniscus tear also occurs when one takes a hard tackle in a soccer game or a sudden pivot while at play in a football field.

A meniscus tear is not just an athlete injury. Even an act as simple as getting up suddenly from a squatting posture can cause a tear in the meniscus. It is on record that no less than 500,000 meniscal tears occur yearly in the United States.

Current Treatment at Other Clinics

Current treatment of knee involves using medications for pain, steroid injection, knee surgery and physical therapy which can temporary alleviate the pain.   These treatments are considered a patch up of the problem without treating the root cause of the problem.  Medications & steroids have many side effects including weight gain & increased blood sugar from steroids, damage to kidneys, heart, stroke & gastric ulcer from NSAIDs.  Surgery can be very debilitating with prolong downtime during recovery requiring at times prolonged bed rest & even residing in nursing home for recovery.  It also entails a lot of risk including risk of infection, bleeding, poor surgical outcome to name a few.  At many times surgery fails to alleviate the pain.  Stem cells however, reduce the inflammation and pain that are typical of these conditions. Stem cell treatment for knee

Platelet rich plasma treatment for knee

Platelets are important in body healing. In PRP, the patient is injected with his or her own platelets to enhance the healing of injured joints, muscles, ligaments and tendons.

To perform a PRP, a sample of your blood is taken and spun in a centrifuge. The platelet fraction is then separated and mixed with a little bit of plasma. This is then injected into the affected tissue. Growth factors are released by the platelets that stimulate the production of reparative cells.

To ensure precision, we use ultrasound imaging to guide the injection process. 

ESWT for knee

During examination, our specialists will locate the point where the pain is most intense, and then apply a water-based medium there. This will enhance the transmission of impulses to that area. A probe will then be placed on that area, and treatment begins.

Very minimal pain or an aching sensation occurs after the treatment. This may last for a few days. After subsequent treatment, you will notice an improvement in symptoms, leading to a reduction in the pain.

Do you use embryonic stem cells?
