Wrist osteoarthritis (OA) is one of the most common conditions encountered by hand surgeons with limited efficacy of non-surgical treatments. The purpose of this study is to describe the Platelet-Rich Plasma (PRP) mixed-microfat biological characteristics of an experimental Advanced Therapy Medicinal Product (ATMP) needed for clinical trial authorization and describe the clinical results obtained from our first three patients 12 months after treatment (NCT03164122). Biological characterization of microfat, PRP and mixture were analysed in vitro according to validated methods. Patients with stage four OA according to the Kellgren Lawrence classification, with failure to conservative treatment and a persistent daily painful condition >40 mm according to the visual analog scale (VAS) were treated. Microfat-PRP ATMP is a product with high platelet purity, conserved viability of stromal vascular fraction cells, chondrogenic differentiation capacity in vitro and high secretion of IL-1Ra anti-inflammatory cytokine. For patients, the only side effect was pain at the adipose tissue harvesting sites. Potential efficacy was observed with a pain decrease of over 50% (per VAS score) and the achievement of minimal clinically important differences for DASH and PRWE functional scores at one year in all three patients. Microfat-PRP ATMP presented a good safety profile after an injection in wrist OA. Efficacy trials are necessary to assess whether this innovative strategy could delay the necessity to perform non-conservative surgery.

Short-term effectiveness of platelet-rich plasma in carpal tunnel syndrome: A controlled study.

Abstract

Platelet-rich plasma (PRP) treatment has a potential to become a part of nonsurgical approach in carpal tunnel syndrome (CTS) as a regenerative method. PRP therapies aim to enhance the self-healing ability of human body, by exposing the injured tissue to a high concentration of autologous growth factors. Nerve tissues also seem to benefit from the regenerative effects of PRP concentrates. The aim of this study is to investigate the possible beneficial effects of PRP injection in CTS. A total of 40 hands of 30 patients were included (20 hands per group) with mild to moderate idiopathic CTS. Patients with mild to moderate CTS were placed into either control or PRP groups. Activity modification and night-only wrist splints were suggested in both groups. Additionally, in PRP group, a single perineural PRP injection into the carpal tunnel was applied under ultrasound guidance. Sensibility tests, Boston carpal tunnel questionnaire (BCTQ), and electrophysiological and ultrasonographical findings were measured initially and after 4 weeks. Groups were similar regarding demographics. BCTQ scores and ultrasonographical values were improved in both groups. Delta analyses revealed that the difference of BCTQ scores improved better in PRP group. Electrophysiological values improved in PRP group. Our study demonstrated that a single, perineural PRP injection into carpal tunnel provided further improvements in CTS.

In the present work, the human bone marrow and adipose tissue-derived mesenchymal stem cells (MSCs) were isolated and expanded under in vitro condition. After their phenotypic analysis, the chondrogenic differentiation was induced by using of the three-dimensional culture system without supplementation of growth factors, and their chondrogenic potential was compared. Obtained results proved that both types of MSCs undergo the process of chondrogenic differentiation. Comparative analysis showed that chondrogenic potential of adipose tissue-derived MSCs was slightly decreased in comparison with bone marrow-derived MSCs. However, both evaluated MSCs may play important role in the cartilage tissue engineering.

Introduction About the Topic

With the advent of keyboards and hand-held devices, there has been an astronomic increased in wear and tear on the hands and wrists. The more we strain our hands, the more they get worn out. Even though this condition may be temporarily salvaged by steroid injections or  total joint reconstruction surgery, complications develop over time especially when the patient does not tackle the biomechanical cause of the injury. Because of this, many patients continue to experience symptoms such as tenderness, numbness, reduced motion range and function. 

Carpal Tunnel Syndrome

The carpal tunnel syndrome is also referred to as median nerve entrapment. It happens during the swelling of a nerve, or inflammation of the tendon.

Symptoms of carpal tunnel syndrome include burning, tingling, itching, and numbness.

If Carpal tunnel syndrome is not treated on time, it may have an adverse effect on the quality of life of the individual. Severe damage to the median nerve may cause permanent numbing of the fingers and weakness of the muscles that were innervated by that particular nerve.

TFCC tear

TFCC is an acronym for triangular fibrocartilage complex (TFCC). This region is located between the radius and the ulna. It consists if many ligaments and tendons. It allows movement of the wrist and also stabilizes the bone of the forearm when the individual grasp something with it, or rotates it.

An injury to this area is what is called a TFCC tear. They are of two types – Type 1 TFCC and Type 2 TFCC tears.

Thumb osteoarthritis

Thumb osteoarthritis is that which affects the hands. People who have thumb osteoarthritis usually experience a break of the thumb cartilage. This allows the bone to come in contact, thus they rub against each other. The symptoms can be very debilitating, mainly because the thumb is one of the most used parts of the body. Complications include reduced grip, and reduction in the range of motion.

Current Treatment at Other Clinics

Current treatment of Hand & Wrist involves using medications for pain, steroid injection, Hand & Wrist surgery and physical therapy which can temporary alleviate the pain.   These treatments are considered a patch up of the problem without treating the root cause of the problem.  Medications & steroids have many side effects including weight gain & increased blood sugar from steroids, damage to kidneys, heart, stroke & gastric ulcer from NSAIDs.  Surgery can be very debilitating with prolong downtime during recovery requiring at times prolonged bed rest & even residing in nursing home for recovery.  It also entails a lot of risk including risk of infection, bleeding, poor surgical outcome to name a few.  At many times surgery fails to alleviate the pain.  Stem cells however, reduce the inflammation and pain that are typical of these conditions. They also work to regenerate and repair the damaged regions 2. Current research on the effect of stem cells in pain management has shown positive results.

Does stem cell therapy have any side effects?
