1. Castagnini F, Pellegrini C, Perazzo L, Vannini F, Buda R. Joint sparing treatments in early ankle osteoarthritis: current procedures and future perspectives. J Exp Orthop. 2016; 3(1):3. doi:10.1186/s40634-016-0038-4

Ankle osteoarthritis (AOA) is a severe pathology, mostly affecting a post-traumatic young population. Arthroscopic debridement, arthrodiastasis, osteotomy are the current joint sparing procedures, but, in the available studies, controversial results were achieved, with better outcomes in case of limited degeneration. Only osteotomy in case of malalignment is universally accepted as a joint sparing procedure in case of partial AOA. Recently, the biological mechanism of osteoarthritis has been intensively studied: it is a whole joint pathology, affecting cartilage, bone and synovial membrane. In particular, the first stage is characterized by a reversible catabolic activity with a state of chondropenia. Thus, biological procedures for early AOA were proposed in order to delay or to avoid end stage procedures. Mesenchymal stem cells (MSCs) may be a good solution to prevent or reverse degeneration, due to their immunomodulatory features (able to control the catabolic joint environment) and their regenerative osteochondral capabilities (able to treat the chondral defects). In fact, MSCs may regulate the cytokine cascade and the metalloproteinases release, restoring the osteochondral tissue as well. After interesting reports of mesenchymal stem cells seeded on scaffold and applied to cartilage defects in non-degenerated joints, bone marrow derived cells transplantation appears to be a promising technique in order to control the degenerative pathway and restore the osteochondral defects.

2. Lee KT, Lee YK, Young KW, Park SY, Kim JS. Factors influencing result of autologous chondrocyte implantation in osteochondral lesion of the talus using second look arthroscopy. Scand J Med Sci Sports. 2012; 22(4):510–5. doi: 10.1111/j.1600-0838.2010.01262.x

This study is to know the factors that may affect cartilage repair after autologous chondrocyte implantation (ACI) for the treatment of an osteochondral lesion of the talus (OLT) as seen through a second look arthroscopy. A total of 38 patients who had ACI treatment for OLT underwent a second look arthroscopy 1 year after the ACI operation. A modified magnetic resonance observation of cartilage repair tissue (MOCART) scoring system was used to assess the outcome of the repaired cartilage. Influencing factors were sex, accompanied procedure, location, site, depth, pre-operative AOFAS score, size and age. Factors that may affect cartilage repair after ACI treatment for OLT were evaluated. Of the different factors assessed, sex (P=0.75), accompanied procedure (P=0.50), depth (P=0.08), location (P=0.54), site (P=0.50) and pre-operative AOFAS score (P=0.42) were found not to affect cartilage repair after ACI treatment for OLT. The size of the lesion (P=0.0021) and patient’s age (P=0.01) were the influential factors. As a result, factors affecting repaired cartilage formation after ACI of OLT were found through the second look arthroscopy. It was determined that not all of the factors affecting the clinical outcome after ACI for the repair of an OLT affected cartilage repair after ACI.

3. Jordan RW, Chahal GS, Chapman A. Is end-stage ankle arthrosis best managed with total ankle replacement or arthrodesis? A systematic review. Adv Orthop. 2014; 2014:986285.

End-stage ankle osteoarthritis is a debilitating condition. Traditionally, ankle arthrodesis (AA) has been the surgical intervention of choice but the emergence of total ankle replacement (TAR) has challenged this concept. This systematic review aims to address whether TAR or AA is optimal in terms of functional outcomes. Methods. We conducted a systematic review according to PRISMA checklist using the online databases Medline and EMBASE after January 1, 2005. Participants must be skeletally mature and suffering from ankle arthrosis of any cause. The intervention had to be an uncemented TAR comprising two or three modular components. The comparative group could include any type of ankle arthrodesis, either open or arthroscopic, using any implant for fixation. The study must have reported at least one functional outcome measure. Results. Of the four studies included, two reported some significant improvement in functional outcome in favour of TAR. The complication rate was higher in the TAR group. However, the quality of studies reviewed was poor and the methodological weaknesses limited any definitive conclusions being drawn. Conclusion. The available literature is insufficient to conclude which treatment is superior. Further research is indicated and should be in the form of an adequately powered randomised controlled trial.

4. Aydin AT, Ozenci M, Gur S. Chondropenia: early stage degenerative disease. Acta Orthop Traumatol Turc. 2007; 41(Suppl 2):19–24

Chondropenia is defined as loss of the articular cartilage volume. It is the early stage of degenerative joint disease. Risk factors include advanced age, obesity, overuse (activity-related injury), menopause, and trauma. Early detection of chondropenia is important with regard to recognition of its causes and assessment of the efficacy of treatment. Magnetic resonance imaging is the most potential tool for noninvasive assessment of articular cartilage. Arthroscopy provides information about the size, surface features, and anatomic localization of cartilage lesions, and the features of the adjacent cartilage. By arthroscopy, tissue thickness and in vivo volume of the cartilage can be measured. Studies have given weight to mechano-acoustic diagnosis and optical tools (optical coherence tomography, reflectance spectroscopy). These studies are based on the relationship between the structural and mechanical features of the cartilage. Determination of breakdown products of cartilage is another potentially important method for assessment.

5. Barg A, Pagenstert GI, Horisberger M, Paul J, Gloyer M, Henninger HB, Valderrabano V. Supramalleolar osteotomies for degenerative joint disease of the ankle joint: indication, technique and results. Int Orthop. 2013; 37(9):1683–95.

Patients with varus or valgus hindfoot deformities usually present with asymmetric ankle osteoarthritis. In-vitro biomechanical studies have shown that varus or valgus hindfoot deformity may lead to altered load distribution in the tibiotalar joint which may result in medial (varus) or lateral (valgus) tibiotalar joint degeneration in the short or medium term. The treatment of asymmetric ankle osteoarthritis remains challenging, because more than half of the tibiotalar joint surface is usually preserved. Therefore, joint-sacrificing procedures like total ankle replacement or ankle arthrodesis may not be the most appropriate treatment options. The shortand midterm results following realignment surgery, are very promising with substantial pain relief and functional improvement observed post-operatively. In this review article we describe the indications, surgical techniques, and results from of realignment surgery of the ankle joint in the current literature.
