1. "Male organ Low Intensity Shock Wave Treatment is Able to Shift PDE5i Nonresponders to Responders: A Double-Blind, Sham Controlled Study" THE JOURNAL OF UROLOGY® – 2016

2. Extracorporeal shockwave therapy for chronic pelvic pain syndrome

Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) is a common disease in males, which is characterized by persistent discomfort or pain in the pelvic region. As currently used drug therapies fail to produce satisfactory results, it is an urgent task to find new and effective methods for the treatment of CP/CPPS. In recent years, many reports are seen on the extracorporeal shock wave therapy (ESWT) for CP/CPPS. ESWT can significantly improve the symptoms of pelvic pain and urination disorders in CPPS patients, and its therapeutic effect is attributed to the improvement of angiogenesis and block of pain nerves. 3. Peyronie's disease and low intensity shock wave therapy: Clinical outcomes and patient satisfaction rate in an open-label single arm prospective study in Australian men

The purpose of this study was to evaluate the efficacy, safety and patient satisfaction outcomes following low intensity extracorporeal shock wave therapy (LiESWT) in men with Peyronie’s disease (PD) using a standardised protocol. In this open-label single arm prospective study, patients with PD were enrolled following informed consent. Patient demographics, change in male organ curvature and plaque hardness, International Index of Erectile Function (IIEF)-5 score, and overall satisfaction score (on a 5-point scale) were recorded. Treatment template consists of 3000 shock waves to the Peyronie’s plaque over 20 minutes, twice weekly for 6 weeks. The majority of patients have PD history longer than 6 months (mean, 12.8 months; range, 6-28 months). Two thirds of patients have received and failed oral medical therapy. There were improvements in male organ curvature (more than 15 degrees in 33% of men), plaque hardness (60% of men) and male organ pain (4 out of 6 men) following LiESWT. There was a moderate improvement in IIEF-5 score (>5 points reported in 20% of men). No complication was reported and the majority of patients were satisfied (rated 4 out of 5; 70% of men) and would recommend this therapy to others.

Vardis and colleagues performed new clinical studies in patients with erectile vasculogenic dysfunction (low blood supply to the male organ). Patients who had poor male organ stiffness were tested after using bluetongue medications. The intent was to investigate the possibility of low-intensity shock wave therapy converting patients who were no longer “functioning” with oral drugs (bluetongue) in “functioning” patients, that is, they would have good male organ stiffness . The study was conducted on 29 patients who had multiple risks for cardiovascular disease, established cardiovascular disease and diabetes. All of them did not present adequate male organ stiffness after oral drug use. After treatment 72% of them had good male organ stiffness, with the use of the oral drug. This increase in stiffness was accompanied by an increase in blood inflow to the male organ and better function of vascular tissues. Another important finding of the study is that no patient reported pain or any adverse events during treatment.

Following this encouraging result other studies have been conducted on animals. Several groups evaluated the effect of low-intensity shock wave therapy on the erectile function of diabetic rats. They performed the evaluation of pressure inside the male organ, after stimulation. It was seen that blood flow in diabetic rats was reduced in all animals, but animals undergoing shock wave therapy had a much lower reduction. In addition to the analysis of the male organ tissues of the animals submitted to shock waves, they had a much larger number of nerves containing nitric oxide (responsible for smooth muscle relaxation inside the male organ, allowing greater blood supply and male organ stiffness), endothelial cells ( are the cells present in blood vessels), smooth muscle cells and mesenchymal stem cells. These findings support the idea that one of the therapeutic mechanisms of the low-intensity shock wave is the recruitment of stem cells that circulate in the blood system.

Another study, done on rats that had the nerves involved, showed that the group undergoing low-intensity shockwave therapy had regeneration of severed nerves, the appearance of new blood vessels, and increased pressure within the male organ. In addition they showed a much larger concentration of progenitor cells (stem cells) in the male organ tissue.”

What Does ESWT/Sonic Wave Therapy Do?

HOW DOES IT WORK?

APPLICATION OF SHOCK WAVE IN ED

As one of the main causes of ED is poor arterial flow, it has been suggested that the induction of neovascularization by the low-intensity shock wave could improve arterial flow, implying an improvement in erectile function. The first study that demonstrated the efficacy of shock wave in ED was performed in 2010 by Vardi et al. The authors applied the low-intensity shock wave in 20 middle-aged patients with mild to moderate ED, who showed improvement with oral medications. They applied the shock wave to 5 different places in the male organ in 6 sessions.

After 1 month of treatment initiation, 75% of the patients had a significant improvement in male organ stiffness. In addition, nocturnal, which occur during REM sleep, were measured and showed a significant improvement in male organ blood flow. Most of the men reviewed after 6 months of treatment continued to have spontaneous reaction without the use of oral drugs.

Shockwave therapy is an innovative treatment involving low-sensitive and mild pulsating sound waves. This therapy is non-invasive and painless. For treatment of chronic pelvic pain syndrome the basic mechanism is via emission of high energy pulses. These energy pulses are targeted at the affected tissues (the pelvis in this case). The technician will use the hand piece of the shockwave therapy machine to emit these pulses. What is extracorporeal shockwave therapy?

Basically, the extracorporeal shockwave therapy is a non-surgical procedure used for the treatment of various disorders such as Achilles tendonitis, tennis elbow, chronic plantar fasciitis, and of course chronic pelvic pain syndrome.

What makes extracorporeal shockwave therapy tick is that it can be used to treat conditions extracorporeally. This means that treatment is done outside the body and not within it. Because of the non-invasive form of treatment, ESWT is less expensive and carries lesser risks than surgery.


Is ESWT for chronic pain syndrome, ED and Peyronie’s disease effective?

Clinical research and experience, including an FDA ESWT study has shown that ESWT is very successful in pain treatment. The FDA study involved 150 subjects. All subjects were asked to rate their pain level using a scale of 1-10. Prior to treatment, the level of pain was 7.7.

A year after the treatment, the patients were asked to rate their level of pain also on a scale of 1-10. Amazingly, patients reported a pain score of 0.6. This indicates a reduction in pain intensity by 92 percent. Apart from this high success rate, ESWT is also high.


What is involved in the ESWT procedure?

Why many patients would benefit from ESWT for chronic pelvic syndrome is because treatment takes place extracorporeally. This means that treatment is done outside of the body. Recovery period is not lengthy, meaning that you will recover fast. This is quite unlike surgery.

During the course of treatment, you will lie on your back. The part to be treated will rest on the machine, on a soft membrane filled with water.
