Umbilical Cord Mesenchymal Stem Cell Treatment for Crohn’s Disease: A Randomized Controlled Clinical Trial

Stem cell therapy has been applied to treat a variety of autoimmune diseases, including Crohn’s disease (CD), but few studies have examined the use of umbilical cord mesenchymal stem cells (UC-MSCs). This trial sought to investigate the efficacy and safety of UC-MSCs for the treatment of CD. Stem cell transplant in inflammatory bowel disease: a promising modality of treatment for a complicated disease course

Inflammatory bowel disease (IBD) is a complex, relapsing and remitting, disease characterized by an exaggerated immune response in a susceptible host. The symptoms and complications of the disease can be debilitating. Advances in medical treatment in the last decade changed the course of the disease in many patients. Despite the use of novel agents for controlling disease, a proportion of patients’ disease courses continue to be either refractory, or become resistant, to available therapeutic options. Stem-cell therapy, with hematopoietic stem cells (HSCs) or mesenchymal stem cells (MSCs), is a promising modality of treatment for severe refractory cases, mainly Crohn’s disease (CD) patients. HSCs have the ability to migrate to damaged tissue, which provides them with further properties to differentiate to epithelial or immune-modulatory cells to restore normal mucosal tissue and integrity. MSCs therapy is a promising model for patients with perianal CD due to their immunosuppressive properties, ability to migrate to areas of injury, and demonstration of colonic healing, including fistulizing tracts. The results from ongoing clinical trials will provide a valuable understanding of the future of stem-cell therapy as a treatment option in refractory cases of IBD, a disease whose pathogenesis remains unknown, and is notoriously difficult to treat. A Brief Look At Crohn’s Disease

Crohn’s disease is a prolonged, or chronic condition, characterized by inflammation of the gastrointestinal tract. Actually, Crohn’s disease is a type of inflammatory bowel disease. It is debilitating, and painful. In some cases, it may be fatal.

Crohn’s disease is also referred to as enteritis or ileitis. It can affect any part of the gastrointestinal tract, beginning from the mouth and extending to the anal region. In most cases, Crohn’s disease occurs in the ileum – that is, the lower aspect of the small intestine.

Frankly, the symptoms of Crohn’s disease are not at all pleasing. These symptoms include pain, discomfort, and ulcers.

A study by the Centers for Disease Control and Prevention has shown that Crohn’s disease affects no less than 26-199 in every 100,000 persons. It mostly begins between the ages of 15-40, but in many cases, can affect anyone at any age.

Symptoms of Crohn’s disease varies depending on the region of the GIT that is affected. The common symptoms include:

  • Pain
  • Anal fissures and bleeding in the rectum
  • Anemia
  • Fatigue
  • Loss of appetite
  • Diarrhea
  • Gut ulcers
  • Mouth ulcers

Other symptoms of Crohn’s disease include:

  • Uvetis (inflammation of the eye)
  • Arthritis
  • Rashes on the skin
  • Inflammation of the liver or bile duct
  • A delay in sexual development in children
  • Delay in growth.

Ulcerative Colitis

Ulcerative colitis is also a chronic condition. It is characterized by inflammation of the large intestine. In some way, ulcerative colitis is similar to Crohn’s disease.

The large intestine, also known as the colon, helps to flush away undigested food and waste substances through the anus.

A person with ulcerative colitis has an inflamed colon. In extreme cases, ulcers are formed on the colon lining. These ulcers produce mucus and pus, and also bleeds.

A study published in the American Family Physician, states that between 250,000 to 500,000 Americans have ulcerative colitis. Sadly, the yearly incidence is 2-7 in every 100,000 persons.

The colon ends at the rectum. The rectum is always affected by ulcerative colitis. If the inflammation occurs in the rectum alone, then the disorder is known as ulcerative procitis. On the other hand, it may also affect the entire colon, in which case it is called pancolitis or universal colitis.

Current Treatment

The current forms of treatment for ulcerative colitis and Crohn’s disease include surgery, medication, and nutritional supplements.

These treatments aim to control the inflammation, relieve the sufferer of symptoms, and also correct nutritional disorders.

  • Anti-inflammation medications – treatment usually begins with mesalamine. Mesalamine helps to control the inflammation.
  • Cortisone or steroids
  • Antibiotics – bacteria may thrive in strictures, or fistulas. This is treated with sulfonamide, ampicillin, tetracycline, cephalosporin, and metronidazole.
  • Anti-diarrheal and fluid replacements

Current Treatment at Other Clinics

Current treatment involves using medications and at times injection.  These treatments are considered a patch up of the problem without treating the root cause of the problem.  Medications & steroids have many side effects including weight gain & increased blood sugar from steroids, damage to kidneys, heart, stroke & gastric ulcer.  Surgery can be very debilitating with prolong downtime during recovery requiring at times prolonged bed rest & even residing in nursing home for recovery.  It also entails a lot of risk including risk of infection, bleeding, poor surgical outcome to name a few.  Stem cells however, reduce the inflammation and pain that are typical of these conditions.
