Stem cell therapy for heart failure

The last decade has witnessed the publication of a large number of clinical trials primarily using bone marrow-derived stem cells as the injected cell. These “first-generation” clinical trials have advanced our understanding and shown us that (1) cell therapy is safe, (2) cell therapy has been modestly effective, and (3) in humans, bone marrow-derived stem cells do not transdifferentiate into cardiomyocytes or new blood vessels (or at least in sufficient numbers to have any effect).

The primary mechanism of action for cell therapy is now believed to be through paracrine effects that include the release of cytokines, chemokines, and growth factors that inhibit apoptosis and fibrosis, enhance contractility, and activate endogenous regenerative mechanisms through endogenous circulating or site-specific stem cells. Cellular therapeutics for heart failure: focus on mesenchymal stem cells

Resulting from a various etiologies, the most notable remains ischemia; heart failure (HF) manifests as the common end pathway of many cardiovascular processes and remains among the top causes for hospitalization and a major cause of morbidity and mortality worldwide. Current pharmacologic treatment for HF utilizes pharmacologic agents to control symptoms and slow further deterioration; however, on a cellular level, in a patient with progressive disease, fibrosis and cardiac remodeling can continue leading to end-stage heart failure. Cellular therapeutics have risen as the new hope for an improvement in the treatment of HF. Mesenchymal stem cells (MSCs) have gained popularity given their propensity of promoting endogenous cellular repair of a myriad of disease processes via paracrine signaling through expression of various cytokines, chemokines, and adhesion molecules resulting in activation of signal transduction pathways. While the exact mechanism remains to be completely elucidated, this remains the primary mechanism identified to date. Recently, MSCs have been incorporated as the central focus in clinical trials investigating the role how MSCs can play in the treatment of HF. In this review, we focus on the characteristics of MSCs that give them a distinct edge as cellular therapeutics and present results of clinical trials investigating MSCs in the setting of ischemic HF. Introduction About the Topic

Cardiac failure and heart failure are synonyms. It is one of the most important ailments that must be treated by a physician. The name of this condition is usually a little confusing. Cardiac or heart failure does not mean a total cessation of heartbeat. What is really going on is that the heart cannot pump blood as well anymore.

The term “cardiac failure” means simply failure of the heart to pump enough blood to satisfy the needs of the body. It is a physiological state in which blood pumped by the heart (cardiac output) is insufficient to meet the needs of the body and lungs.

Cardiac failure can result from any heart condition that reduces the ability of the heart to pump blood. The cause is usually decreased contractility of the myocardium resulting from a reduced coronary blood flow. Common causes also include high blood pressure, atrial fibrillation, valvular heart disease, excess alcohol use, infection, and cardiomyopathy of an unknown cause. External pressure around the heart and vitamin B deficiency also contribute to making the heart a hypo-effective pump.

If a heart suddenly fails, the pumping ability of that heart gets depressed. The chambers of the heart may then respond by stretching to carry more blood to pump through the body. They may become stiffer and thicker. This helps keep blood moving for a while, but in time, the heart muscle walls may get weaker. The kidneys react by causing the body to hold on to water and salt. Fluid may start to build up in the arms, legs, ankles, feet, lungs, or other organs.

Factors that cause heart failure do so by changing either the structure or the functioning of the heart.

There are two main types of heart failure:

 

  • Heart failure due to left ventricular dysfunction
  • Heart failure with normal ejection fraction depending on whether the ability of the left ventricle to contract or relax is affected or not.

Usually, people with heart failure experience difficulties during exercise. This is a yardstick to grade the severity of the disease. It should be noted that there are dissimilarities between heart failure, myocardial infarction or cardiac arrest. While heart failure means an inability of the heart to pump sufficient quantity of blood for the body’s use, myocardial infarction describes a condition in which part of the heart muscle dies, while cardiac arrest is a condition in which blood flow stops altogether. Smoking and a sedentary lifestyle are risk factors for heart failure.

Epidemiology

Heart failure is a common, costly and potentially fatal condition. There are reports that about 40 million people were affected globally in 2015. In developed countries, around 2% of patients have heart failure and in those over the age of 65, this increases to 6 to 10 percent. The risk of death increases about 35 percent a year after diagnosis after which it decreases to below 10 percent each year. This is similar to the risks with a number of types of cancer. In the United Kingdom the disease is the reason for five percent of emergency hospital admissions. Heart failure has been known since ancient times, with the earliest records dating as far back as 1550 BCE.

Symptoms And Diagnosis

Signs and symptoms commonly include shortness of breath, excessive tiredness, and leg swelling due to edema. The shortness of breath is usually worse with exercise, while lying down, and may wake the person at night. A limited ability to exercise is also a common feature. A few diseases share symptoms similar to heart failure. They include obesity, kidney failure, liver problems, and anemia and thyroid disease.

Diagnosis of this condition is based on the history of the symptoms and a physical examination by a doctor. Confirmation is done by echocardiography (ECHO).

 

Current Treatment at Other Clinics

Current treatment involves using medications and at times injection.  These treatments are considered a patch up of the problem without treating the root cause of the problem.  Medications & steroids have many side effects including weight gain & increased blood sugar from steroids, damage to kidneys, heart, stroke & gastric ulcer.  Surgery can be very debilitating with prolong downtime during recovery requiring at times prolonged bed rest & even residing in nursing home for recovery.  It also entails a lot of risk including risk of infection, bleeding, poor surgical outcome to name a few.  Stem cells however, reduce the inflammation and pain that are typical of these conditions. Current ongoing research on the effect of stem cells has shown positive results.

We are different from many other clinics who offer stem cell or PRP.   Other may also not offer option of bone marrow derived stem cell, Adipose derived stem cell & umbilical cord stem cell.  They in effect expose patients to more invasive treatments like drilling bones without offering other easier options.  They also may use xray with unnecessary radiation for injections that can be easily avoided in most cases by using ultrasound.  Furthermore, they may not have proper knowledge of how other modalities can enhance the effects of Stem Cells.  These other modalities are used by us to focus on the whole body and treating different parts of it instead of just treating one out of several diseased structures in the body.

As a matter of fact, a very low percentage of donated umbilical cords scale through this screening process.

From where do stem cells originate?

Stem cells are present in many tissues and organs. The bone marrow is the richest source of stem cells. However, the most convenient region for harvesting is the umbilical cord.
