1. Stem cell therapy in neurodegenerative diseases

The lack of curative therapies for neurodegenerative diseases has high economic impact and places huge burden on the society. The contribution of stem cells to cure neurodegenerative diseases has been unraveled and explored extensively over the past few years. Beyond substitution of the lost neurons, stem cells act as immunomodulators and neuroprotectors. A large number of preclinical and a small number of clinical studies have shown beneficial outcomes in this context. In this review, we have summarized the current concepts of stem cell therapy in neurodegenerative diseases and the recent advances in this field, particularly between 2010 and 2012. Further studies should be encouraged to resolve the clinical issues and vague translational findings for maximum optimization of the efficacy of stem cell therapy in neurodegenerative diseases. 2. Stem cells for the treatment of neurodegenerative diseases

Stem cells offer an enormous pool of resources for the understanding of the human body. One proposed use of stem cells has been as an autologous therapy. The use of stem cells for neurodegenerative diseases has become of interest. Clinical applications of stem cells for Alzheimer disease, Parkinson disease, amyotrophic lateral sclerosis, and multiple sclerosis will increase in the coming years, and although great care will need to be taken when moving forward with prospective treatments, the application of stem cells is highly promising. 3. Clinical safety of intrathecal administration of mesen-chymal stromal cell-derived neural progenitors in multiple sclerosis.

RESULTS:

Six patients with progressive MS were treated with between 2 and 5 intrathecal injections of escalating doses of autologous MSC-NPs and were followed an average of 7.4 years after initial injection. There were no safety concerns noted, no serious adverse events, and the multiple dosing regimen was well tolerated. Four of the six patients showed a measurable clinical improvement following MSC-NP treatment.

DISCUSSION:

This pilot study supports preliminary first-in-human safety and tolerability of autologous MSC-NP treatment for MS.

The potential of human umbilical cord-derived mesenchymal stem cells as a novel cellular therapy for multiple sclerosis

Abstract

Multiple sclerosis (MS) is a complex disease of neurological disability, affecting more than 300 out of every 1 million people in the world. The purpose of the study was to evaluate the therapeutic effects of human umbilical cord-derived mesenchymal stem cell (hUC-MSC) transplantation in MS patients. Twenty-three patients were enrolled in this study, and 13 of them were given hUC-MSC therapy at the same time as anti-inflammatory treatment, whereas the control patients received the anti-inflammatory treatment only. Treatment schedule included 1,000 mg/kg of methylprednisolone intravenously (IV) daily for 3 days and then 500 mg/kg for 2 days, followed by oral prednisone 1 mg/kg/day for 10 days. The dosage of prednisone was then reduced by 5 mg every 2 weeks until reaching a 5-mg/day maintenance dosage. Intravenous infusion of hUC-MSCs was applied three times in a 6-week period for each patient. The overall symptoms of the hUC-MSC-treated patients improved compared to patients in the control group. Both the EDSS scores and relapse occurrence were significantly lower than those of the control patients. Inflammatory cytokines were assessed, and the data demonstrated a shift from Th1 to Th2 immunity in hUC-MSC-treated patients. Our data demonstrated a high potential for hUC-MSC treatment of MS. This manuscript is published as part of the International Association of Neurorestoratology (IANR) special issue of Cell Transplantation.

Introduction About the Topic

Degenerative diseases are diseases resulting from cell damage. They affect organs or tissues, causing it to deteriorate gradually over time.

Degenerative diseases also affect the central nervous system. Here, the central nervous system fails to work the way it should. One of this is Alzheimer’s disease. Other types of degenerative diseases are those affecting the circulatory system (such as coronary artery disease) and neoplastic diseases (like cancer).

There are many degenerative diseases in existence. Some of these diseases are related to aging. Degenerative diseases may be worsened by certain lifestyle choices or normal bodily wear, this, however, depends on the disease. In some cases, the cause of the disease may be genetic. Thus, some degenerative diseases are hereditary. Sometimes, it may be attributed to viral causes, chemicals or poisons. Some may be of unknown causes.

Multiple sclerosis

Multiple sclerosis is a disease of the central nervous system. However, the main cause of this disease is unclear.

The nerve fibers of the central nervous are coated with a myelin sheath. This sheath offers protective functions. The myelin also facilitates the conduction of electrical signals. In multiple sclerosis, there is conspicuous absence of the sheath, leaving behind a sclerosis, or scar.

Areas without myelin sheath are known as lesions or plaques. As these plaques worsen, the nerve fibers get damaged or break off. Because of this, the electrical impulses are not efficiently conveyed from the brain to the target nerve.

In the absence of myelin, the fibers are unable to conduct electrical impulses. Thus, the brain becomes unable to transmit messages from the brain to the muscles.

Amyotrophic lateral sclerosis

Amyotrophic lateral sclerosis is a kind of motor neuron disease. In this disease, nerve cells that help in the performance of voluntary actions are attacked.

Motor neurons are present in the brain and in the spinal cord. In amyotrophic lateral sclerosis, these nerve cells degenerate and die. They become unable to transmit impulses to the muscles. The brain then fails to control voluntary movement resulting in weakening and wasting away of the muscles.

Progression of amyotrophic lateral sclerosis leads to deterioration of the voluntary muscles. The affected individual is unable to control the legs, face, and the arms. With time, the individual suffers respiratory failure due to the inability to breathe.

Research has shown that about 50 percent of ALS patients will live for at least three years after diagnosis. Some live much longer.

Other types of degenerative diseases include:

  • Muscular dystrophy
  • Diabetes
  • Rheumatoid arthritis

Current Treatment at Other Clinics

Current treatment involves using medications and at times injection.  These treatments are considered a patch up of the problem without treating the root cause of the problem.  Medications & steroids have many side effects including weight gain & increased blood sugar from steroids, damage to kidneys, heart, stroke & gastric ulcer.  Surgery can be very debilitating with prolong downtime during recovery requiring at times prolonged bed rest & even residing in nursing home for recovery.  It also entails a lot of risk including risk of infection, bleeding, poor surgical outcome to name a few.  Stem cells however, reduce the inflammation and pain that are typical of these conditions. Current ongoing research on the effect of stem cells has shown positive results.

From where do stem cells originate?

How are adult stem cells harvested?
