1. Platelet-rich plasma for the management of hip and knee osteoarthritis

Knee and hip osteoarthritis (OA) are major public health problems worldwide causing pain, disability and impaired quality of life. 2. Mesenchymal stem cell therapy in the treatment of hip osteoarthritis

This study was performed to investigate the safety and efficacy of the intra-articular infusion of ex vivo expanded autologous bone marrow-derived mesenchymal stem cells (BM-MSC) to a cohort of patients with articular cartilage defects in the hip. The above rationale is sustained by the notion that MSCs express a chondrocyte differential potential and produce extracellular matrix molecules as well as regulatory signals that may well contribute to cure the function of the damaged hip joint. A cohort of 10 patients with functional and radiological evidences of hip osteoarthritis, either in one or both legs, was included in the study. BM-MSC (the cell product) were prepared and infused into the damaged articulation(s) of each patient (60 × 106 cells in 3 weekly/doses). Before and after completion of the cell infusion scheme, patients were evaluated (hip scores for pain, stiffness, physical function, range of motion), to assess whether the infusion of the respective cell product was beneficial. The intra-articular injection of three consecutive weekly doses of ex vivo expanded autologous BM-MSC to patients with articular cartilage defects in the hip and proved to be a safe and clinically effective treatment in the restoration of hip function and range of motion. In addition, the statistical significance of the above data is in line with the observation that the radiographic scores (Tönnis Classification of Osteoarthritis) of the damaged leg(s) remained without variation in 9 out of 10 patients, after the administration of the cell product. 3. High-energy extracorporeal shock wave therapy for nontraumatic osteonecrosis of the femoral head

Nontraumatic osteonecrosis of the femoral head (ONFH) is treated with a series of methods. High-energy extracorporeal shock wave therapy (ESWT) is an option with promising mid-term outcomes. The objective of this study was to determine the long-term outcomes of ESWT for ONFH. Fifty-three hips in 39 consecutive patients were treated with ESWT in our hospital between January 2005 and July 2006. Forty-four hips in 31 patients with stage I-III nontraumatic ONFH, according to the Association Research Circulation Osseous (ARCO) system, were reviewed in the current retrospective study. The visual analog pain scale (VAS), Harris hip score, radiography, and magnetic resonance imaging were used to estimate treatment results. The progression of ONFH was evaluated by imaging examination and clinical outcomes. The results were classified as clinical success (no progression of hip symptoms) and imaging success (no progression of stage or substage on radiography and MRI). The results of the current study indicated that ESWT is an effective treatment method for nontraumatic ONFH, resulting in pain relief and function restoration, especially for patients with ARCO stage I-II ONFH. 4. Extracorporeal shockwave for hip necrosis in systemic lupus erythematosus.

Patients with systemic lupus erythematosus (SLE) frequently received corticosteroid therapy, resulting in osteonecrosis of the femoral head (ONFH). Prior studies demonstrated the effectiveness of extracorporeal shockwave treatment (ESWT) for ONFH. This study evaluated the effectiveness of ESWT for ONFH in patients with SLE. We studied 39 patients, including 15 patients with SLE (26 hips) and 24 controls (29 hips). To each affected hip we applied ESWT (6000 impulses at 28 kV in a single session). Patients were ambulated with partial weight bearing for 4-6 weeks. The primary endpoint was the need for hip replacement. The secondary endpoints were improvement in hip pain and function and image changes on X-ray and MRI. Patients received total hip replacement in 12% of patients with SLE and in 14% of controls (P = 0.802). There was no statistically significant difference in pain scores (0.86 vs. 0.89; P = 0.467) and function scores (89% vs. 91%; P = 0.194) between patients with SLE and controls. Hip labral tear

Our ball-and-socket joints are surrounded by a type of cartilage known as the labrum. It is readily found in the joints of the hip and the shoulder. The labrum makes the joint stable by deepening the socket, and also allowing flexibility and motion.

Labral injuries causes a lot of discomfort and pain. Labral injuries occur mostly in the shoulders. Lots of research has been done on the possible treatments of labral injuries. With advances in arthroscopic techniques, for surgical management of the hip joint, more people get to know about hip labral tears.

Hip labral tears are of two types- we have the degenerative tear and the traumatic tear.

The degenerative tear occurs due to persistent use. It is very evident in the initial stage of hip arthritis.

On the other hand, the traumatic tear is an acute condition that occurs during a fall, an accident, or a sports injury.

Hip labral tears are also seen during subluxation or hip dislocation.

Hip osteoarthritis

Osteoarthritis is one of the major forms of hip arthritis. It is also referred to as degenerative joint disease or wear-and-tear arthritis. The major feature of hip arthritis is progressive degradation of the joint cartilage. As the cartilage wears away, the bare bone gets exposed.

Hip osteoarthritis occurs mainly in people above 50 years of age. It has a high prevalence among obese patients. Symptoms are alleviated by weight loss. Genetics also contributes to it. This means that an individual with a family history of the disease is likely to get it. Other factors that play a role in the onset of hip osteoarthritis include fractures and injuries to the hip.

Hip tendon tears

There are quite a number of tendons in the hip region. These tissues establish a connection between muscles and hip bones. With these, movements of the hip and the leg are stabilized. Tendons are connective tissues, but non-elastic. Frequent use and aging makes it to become brittle. Repetitive use of the tendon causes injury. Injury also arises when a blow is done to the hip. It may also result from injuries that are not fully healed. Athletes have a high risk of hip tendon tears.

Current Treatment at Other Clinics

Current treatment of hip involves using medications for pain, steroid injection, hip surgery and physical therapy which can temporary alleviate the pain.   These treatments are considered a patch up of the problem without treating the root cause of the problem.  Medications & steroids have many side effects including weight gain & increased blood sugar from steroids, damage to kidneys, heart, stroke & gastric ulcer from NSAIDs.  Surgery can be very debilitating with prolong downtime during recovery requiring at times prolonged bed rest & even residing in nursing home for recovery.  It also entails a lot of risk including risk of infection, bleeding, poor surgical outcome to name a few.  At many times surgery fails to alleviate the pain.  Stem cells however, reduce the inflammation and pain that are typical of these conditions. Platelet rich plasma therapy for hip

PRP begins with drawing a small sample of your blood. This is then placed in a centrifuge and spun. The centrifugation process separates the blood into different fragments – the platelets, red cells, white cells, and the plasma. What determines the outcome of my therapy?

The outcome of your therapy is determined by various factors, like the extent of damage, the location of treatment, and disease. Most patients have a positive response to this therapy and experience relief from pain within a short period of time.

How do we collect the cells?

The answer is amniotic membrane allograft or cord blood. We collect the cells from mother’s who have donated their placental tissue after delivery. We can also obtain stem cells from your bone marrow or adipose tissue by a mini liposuction.

Does stem cell therapy have any side effects?

Side effects are very minimal. It may be accompanied by minor irritation or pain at the site of injection. Because we are using your own stem cells, the chances of rejection by your immune system are extremely low.
