Effect of Autologous Cord Blood Infusion on Motor Function and Brain Connectivity in Young Children with Cerebral Palsy: A Randomized, Placebo‐Controlled Trial

Jessica M. Sun Allen W. Song Laura E. Case Mohamad A. Mikati Kathryn E. Gustafson Ryan Simmons  Ricki Goldstein Jodi Petry

Abstract

Cerebral palsy (CP) is a condition affecting young children that causes lifelong disabilities. Umbilical cord blood cells improve motor function in experimental systems via paracrine signaling. After demonstrating safety, we conducted a phase II trial of autologous cord blood (ACB) infusion in children with CP to test whether ACB could improve function (ClinicalTrials.gov, NCT01147653; IND 14360). In this doubleblind, placebocontrolled, crossover study of a single intravenous infusion of 1–5 × 107 total nucleated cells per kilogram of ACB, children ages 1 to 6 years with CP were randomly assigned to receive ACB or placebo at baseline, followed by the alternate infusion 1 year later. Motor function and magnetic resonance imaging brain connectivity studies were performed at baseline, 1, and 2 years posttreatment. The primary endpoint was change in motor function 1 year after baseline infusion. Additional analyses were performed at 2 years. Sixtythree children (median age 2.1 years) were randomized to treatment (n = 32) or placebo (n = 31) at baseline. Although there was no difference in mean change in Gross Motor Function Measure66 (GMFM66) scores at 1 year between placebo and treated groups, a dosing effect was identified. In an analysis 1 year postACB treatment, those who received doses ≥2 × 107/kg demonstrated significantly greater increases in GMFM66 scores above those predicted by age and severity, as well as in Peabody Developmental Motor Scales2 Gross Motor Quotient scores and normalized brain connectivity. Results of this study suggest that appropriately dosed ACB infusion improves brain connectivity and gross motor function in young children with CP. Stem Cells Translational Medicine2017;6:2071–2078

Treatment of Cerebral Palsy with Stem Cells: A Report of 17 Cases

Cerebral Palsy (CP) is a disabling condition that affects a child’s life and his/her family irreversibly. It is usually a non-progressive condition but improvement over time is rarely seen. The condition can be due to prenatal hypoxia, metabolic, genetic, infectious, traumatic or other causes. It is therefore a heterogeneous group that results in functional motor disability associated with different degrees of cognitive abnormalities. There are no treatments that can cure or even improve CP and the best available approach aims at functional, social and nutritional supportive care and counseling. In this paper, we report 17 sequential patients with CP treated with intrathecal administration of Bone Marrow Mononuclear Cells (BMMC). All patients had an uneventful post-injection course with 73% of the evaluable patients treated having a good response using the Gross Motor Function Classification System (GMFCS). The average improvement was 1.3 levels on the GMFCS with cognitive improvements as well. Link to the study >>>>> HERE

Concise Review: Stem Cell Interventions for People with Cerebral Palsy: Systematic Review with Meta-Analysis

Evidence for stem cells as a potential intervention for cerebral palsy is emerging. Our objective was to determine the efficacy and safety of stem cells for improving motor and cognitive function of people with cerebral palsy. Searches were conducted in October 2015 in CENTRAL, EMBASE, MEDLINE, and Cochrane Libraries. Randomized controlled trials and controlled clinical trials of stem cells for cerebral palsy were included. Two authors independently decided upon included trials, extracted data, quality, and risk of bias. The primary outcome was gross motor function. Secondary outcomes were cognitive function and adverse events (AEs). Effects were expressed as standardized mean differences (SMD) with 95% confidence intervals (CI), using a random-effects model. Five trials comprising 328 participants met inclusion criteria. Four cell types were studied: olfactory ensheathing, neural, neural progenitors, and allogeneic umbilical cord blood (UCBs). Transplantation procedures differed from central nervous system neurosurgical transplantation to intravenous/arterial infusion. Participants were followed short-term for only 6 months. Evidence of variable quality indicated a small statistically significant intervention effect from stem cells on gross motor skills (SMD 1.27; 95% CI 0.22, 2.33), with UCBs most effective. There were insufficient and heterogeneous data to compare cognitive effects. Serious AEs were rare (n = 4/135 [3%] stem cells; n = 3/139 [2%] controls). Stem cells appeared to induce short-term improvements in motor skills. Different types of stem cell interventions were compared, meaning the data were heterogeneous and are a study limitation. Further randomized controlled trials are warranted, using rigorous methodologies. Link to the study >>>>> HERE

Introduction About the Topic

What is cerebral palsy?

Cerebral Palsy is a group of disorders affecting movement and coordination of the muscles. Most cases of cerebral palsy also affect hearing, sensation, and vision.

Cerebral means “brain.” Palsy on the other hand means “issues with body movement or weakness.”

Most cases of movement disorders in childhood are attributed to cerebral palsy. A study by the Centers for Disease Control and Prevention has shown that at least 1 in every 1,000 children globally are affected by cerebral palsy.

How do i know that i have cerebral palsy? What are the symptoms?

The symptoms of cerebral palsy vary from person to person. Furthermore, these symptoms ranges from mild to severe. Some patients with the disorder may find it difficult to sit or to walk. Other people may encounter issues with grasping objects.

The symptoms of cerebral palsy may get more complicated or less severe with time. Symptoms also vary depending on the affected part of the brain.

Common symptoms of cerebral palsy include the following;

  • Very stiff or floppy muscle tone
  • Delay in attaining motor skill milestones, for instance, crawling, and sitting up without help, or rolling over.
  • Difficulty in speaking, or language delay
  • Inability to coordinate the muscle – known as ataxia
  • Spasticity or stiffness of the muscle
  • Difficulty walking
  • Involuntary movements or tremors
  • Neurological disorders such as blindness, seizures, and intellectual disability.

Most children have cerebral palsy at birth, but may not show the symptoms until months or years later. In most cases, the symptoms manifest before the child attains the age of 3 or 4.

What is the main cause of cerebral palsy?

Cerebral palsy may be cause by an injury to the developing brain, or failure of the brain to develop properly. Parts of the brain that are usually affected include parts controlling coordination, posture, and body movement.

Brain damage occurs before the baby is born, but in some cases, may also be caused after birth. Frankly, the exact cause of most cases of cerebral palsy is not known. Some factors that may contribute to this disorder include;

  • Jaundice in the baby
  • Asphyxia neonatorum, or poor oxygen supply to the brain (during childbirth)
  • Genetic mutations that result in abnormal development of the brain
  • Infections of the brain, such as meningitis and encephalitis
  • Injuries to the head due to automobile accidents, child abuse, or a fall.
  • Intracranial hemorrhage

Current Treatment at Other Clinics

There are many treatment options for cerebral palsy. These include physical therapy, speech therapy, and occupational therapy. Other forms of treatment include Botox injections, surgery, and hyperbaric oxygen therapy.

Current treatment at other clinics involves using medications and at times injection.  These treatments are considered a patch up of the problem without treating the root cause of the problem.  Medications & steroids have many side effects including weight gain & increased blood sugar from steroids, damage to kidneys, heart, stroke & gastric ulcer.  Surgery can be very debilitating with prolong downtime during recovery requiring at times prolonged bed rest & even residing in nursing home for recovery.  It also entails a lot of risk including risk of infection, bleeding, poor surgical outcome to name a few.  Stem cells however, reduce the inflammation and pain that are typical of these conditions. Current ongoing research on the effect of stem cells has shown positive results.

We are different from many other clinics who offer stem cell or PRP.   Other may also not offer option of bone marrow derived stem cell, Adipose derived stem cell & umbilical cord stem cell.  They in effect expose patients to more invasive treatments like drilling bones without offering other easier options.  They also may use xray with unnecessary radiation for injections that can be easily avoided in most cases by using ultrasound.  Furthermore, they may not have proper knowledge of how other modalities can enhance the effects of Stem Cells.  These other modalities are used by us to focus on the whole body and treating different parts of it instead of just treating one out of several diseased structures in the body.

Safe Non Surgical Option

We use a combination of newest cutting edge technology in regenerative medicine to stimulate body’s own repair ability to reverse aging and damage.   We use Stem Cells as part of treatment plan designed by the doctor to treat the body.    These are safe investigational treatments that have shown very promising & safe results in thousands of treatments over the 10-20 years in Europe and USA.   They don’t involve using any medications or drugs.

What Does Stem Cell Therapy Do?

The researchers believe that the therapy works by:

Releasing cytokines into the body to prevent further damage and inflammation.

Preventing inflammation that can make the disease worse.

Creating healthy new tissues or missing or damaged products in body.

Many patients experience improvements in the treated area and other areas, like in a Mayo Clinic study where patients benefited in both knees after experiencing injections in only one.

Can cerebral palsy be treated with stem cells?

Yes! Treatment is done with umbilical cord mesenchymal stem cells. These cells are widely accepted for therapeutic purposes because the body does not readily identify them as foreign cells. These cells repair damaged tissues and also secrete trophic factors.

Trophic factors boost the repair of brain and spinal cord tissues. It induces angiogenesis (development of new blood vessels), neurogenesis, and activation of astroglial cells. They enhance anxonal remyelination and synaptic connection, decrease apoptosis, and of course regulate activation of microglial cells.

Umbilical cord mesenchymal stem cells stimulate repair of the brain cells after traumatic brain injury and stroke.

Studies conducted on animals have shown that mesenchymal stem cells can move into the brain and live.

Because our facility makes use of allogenic cord-derived mesenchymal stem cells, we can treat any patient, not only those who had saved their cord blood at birth.

Clinical research has shown that mononuclear stem cells derived from the bone marrow are very safe and can boost the quality of life in patients with cerebral palsy.

What makes our facility special?

At our Los Angeles facility, we have invented modern approaches to medical care. Our approach to treatment is second to none. The harvesting process does not involve any form of drilling, and the stem cells are purified and tested for quality before injection. We also employ ultrasound to help in the precise location of high dose stem cells at the required site. These cells will then perform their duty, promoting the formation of new tissues. We are experts in this and has seen positive outcome with our treatments. Our licensed physicians have undergone intense training running into thousands of hours, and so, you can be sure that you are in safe hands.

Frequently Asked Questions

What is cerebral palsy?

Cerebral palsy is a group of neurological disorders that affect movement and muscle coordination.

What causes cerebral palsy?

The major cause of cerebral palsy is injury or damage to the brain, whether due to developmental, genetic, or disease.

What kind of communication aid can help a patient with cerebral palsy to communicate with others or to move around?

There are a wide range of equipment that can help CP patients to communicate or move around. Some of the devices that can be used by CP patients include:

  • Scooters
  • Wheelchairs
  • Crutches and walkers
  • Customized tricycles and bicycles

Communication devices that can be used by CP patients are:

  • Voice synthesizers
  • Symbol boards
  • Keyguards and head sticks for computers.
Does cerebral palsy affect everyone the same way?

No. Patients are affected in different ways. Cerebral palsy has many causes. It is therefore not surprising that it takes so many forms. Each CP patient is a unique personality, but may likely be classified as having a particular type of cerebral palsy. The patient may be classified according to movement disorder or the number of limbs that are affected.

Can cerebral palsy be transferred to another person? Is it contagious?

No! Cerebral palsy is not contagious. As a matter of fact, cerebral palsy is not a disease. It is a condition.

What are the treatment options for cerebral palsy?

There are many treatment options for cerebral palsy. These include physical therapy, speech therapy, and occupational therapy. Other forms of treatment include Botox injections, surgery, and hyperbaric oxygen therapy. New and more effective forms of treatment include stem cell injections.

What medications can be used to treat cerebral palsy in children?

Medications may be prescribed for conditions that are associated with cerebral palsy, like seizures for instance. In some cases, medications may also be prescribed for painful spasms or spasticity. Nerve blockers can be used to reduce spasticity (although temporarily).

How many people have cerebral palsy?

Studies have shown that one in every five hundred babies have cerebral palsy. It is also estimated that at least one in three premature born are affected by cerebral palsy.

From where do stem cells originate?

Stem cells are present in many tissues and organs. The bone marrow is the richest source of stem cells. However, the most convenient region for harvesting is the umbilical cord.

How are adult stem cells harvested?

The physician draws the stem cells from the umbilical cord. The stem cells and the growth factors will then be concentrated using specially designed equipment. The physician then injects the cells into the destined regions.

What’s the Cost of Stem Cell Injection?

The cost of treatment depends on the procedure performed.  We often offer discounts for returning customers or larger packages of treatment using repeated injections or combining different treatments.  We also offer a price match policy on comparing treatments done by board certified physicians.  We also have interest free payment plan options including care credit for qualifying patients. A limited number of insurance companies and Workers compensation may at times cover some of the expenses.  You may be able to get reimbursed by your insurance for your doctor visits & ultrasound cost. The consultation fee can be deducted from your future procedures, making your consolation free of cost to you.

Book a consultation with our experts to see the type of procedure that is right for you.

When will my results show up?

Stem cell therapy relies on your own regenerative healing to occur. We are all different and so is our healing process. Some patients may begin to see their results within few weeks while others may require some months. The time taken for healing to manifest also depends on the severity of the disease.

What can stem cell therapy potentially treat?

Stem cell therapy is beneficial to a number of conditions such as osteoarthritis, erectile dysfunction, joint pain, renal disease, neurological disorders, autism, cellular rejuvenation, etc.

How are stem cells administrated?

We use different methods of delivery based on the condition & symptoms.  We are able to deliver the stem cells using, ultrasound guided injections directly to the site, IV or intervenes, nebulized or intrathecal according to the condition and decision of the doctor.   We usually employ the least invasive & safest method of delivery.
